2. (OPTIONAL) Orlando Trip Registration form - pay $360 fee

3. DRAGONFLY ACCOUNT

• You will upload your Physical Form, Medical Insurance Card, E-sign all Fulton County forms, etc.  Note:  The Athlete Fee will be due once on the official roster, DO NOT PAY THE $250 until you've made the team (on roster).
• Dragonfly Account School Code: MLMXHC - Create/Update your own account (instructions here).  
• Mandatory Physical Form (click here to download) filled out by medical professional.  It is good for one year.
