MANDATORY PRIVIT PROFILE

All athletes must complete the Privit online athlete profile to participate in sports at Northview High School. For instructions how to create your profile, go to http://northviewtitans.org.  Click on MORE for instructions.

MANDATORY ATHLETE PHYSICAL FORM

Each athlete must have a physical form completed by a physician on file. The form is good for one year. Go to http://northviewtitans.org. Click on MORE to find a copy of the Pre-Participation Physical form.

Note, all athletes participating in sports must upload forms to the Privit system and complete the Privit registration to participate.
